How to prepare for my appointment
To help prepare for your appointment please ensure you have your appointment letter, which will include the hospital or community clinic you need to attend, the date and time of your appointment, and any other relevant information you may be asked to bring with you.
Planning your journey and parking/transport
- Before you travel to your appointment ensure you know which hospital or community clinic you are attending, where you are going to park/transport arrangements, and allow sufficient time to get to your appointment, find parking and book in at the reception area detailed on your letter.

What do I need to bring to the appointment?
- Your appointment letter or appointment card.
- Information about you; including your address, your GP’s address and for children, their school address.
- Medicines and allergy treatments; the names of the medication, what you’re taking it for and how often you take it.
- A list of any allergies you suffer from.
- If you are coming from a nursing/care home then please bring your care plan.
- Immunisation history, and any relevant previous medical history.
- Any relevant family medical history.
Bringing a relative or friend to my appointment
- Outpatients can be accompanied at their appointment by a relative or friend if needed. We do ask that you bring only one person with you, if possible.
- There may be situations in the clinic setting where a relative/friend is asked to wait outside the clinic room whilst a consultation takes place.
