Long term management
It is important that by the end of the cardiac rehabilitation programme, you should be empowered to participate in physical activity for the long term. This can be achieved through an exit interview where physical activity plans are agreed and arrangements can be made to join local services that will continue to support you. The aim of this process is to develop your confidence to exercise independently and to take responsibility for your health to pursue a healthy lifestyle.
Patients will be signposted and encouraged, where appropriate, to join:
- Heart support groups
- Community exercise and activity groups
- Tobacco and smoking cessation services

BHF training guides – Beginner, intermediate, advanced guides for walking, running, cycling, swimming, and triathlon.
