Heart Function Service
The Heart Function team are experienced nurses who have undertaken extended training in managing heart function problems. They work closely with the consultant cardiologists and other health care teams.
We aim to help you and your family and/or carer to:
- Access education and ongoing support and monitoring
- Benefit from evidence-based medications, procedures & rehabilitation
- Improve your confidence
Improve your quality of life - Avoid hospital admissions
- Shorten any stays in hospital
Who can benefit from a consultation with a Heart Function Nurse?
You can benefit if you have been told by a doctor that you have Left Ventricular Systolic Dysfunction (LVSD) or heart failure. This should have been confirmed by a test called an echocardiogram.
Family members and/or carers would also benefit from a consultation as this would help them to support you.
Where can I see a Heart Function Nurse Specialist?
The nurses consult with patients on the wards at the Hereford County Hospital and in their own nurse-led outpatient clinics. They also offer a telephone follow-up clinic and rehabilitation sessions.
Do I need a referral?
If you are a patient on the ward we can offer you a consultation during your admission, please ask the ward staff to contact us by telephone as soon as possible.
If you attend the cardiology clinic, please ask the clinic doctor to refer you. If not, then please ask your GP to refer you to the cardiology team for the Heart Function team.
