How does PIFU work?
Your clinician may recommend the PIFU pathway if it is safe for you to manage your own follow-up appointments. Once you and your clinician agree you should go onto a PIFU pathway, your clinician will discuss with you the signs and symptoms of when your Rheumatology condition is active, which will also be highlighted in your clinical letter.
You should contact us to arrange an appointment if you experience any of those signs and symptoms.
If you are put on a PIFU transfer pathway, at the end of the agreed timeframe, you will receive an appointment. If you do not need the appointment, please contact us and, if appropriate, we can postpone it.
- If you are put on a PIFU discharge pathway, at the end of the agreed timeframe, you will be discharged to you GP.
Please refer to your clinical letter for more information about your PIFU plan and period.
Managing your appointments in this way is optional and it is your decision.
