Speech and Language Therapy team
If you, or a relative of yours, have communication and / or swallowing difficulties resulting from surviving a stroke, then you may be referred to the Community Stroke Service (CSS).
The speech and language therapy team that works as part of the CSS provides specialist rehabilitation within the person’s home environment. They advise and support people with communication and / or swallowing difficulties resulting from a stroke providing there has been a confirmed diagnosis of stroke within the last year.
Following a period of assessment, therapy plans are agreed with the person and their families. This plan will include how often the therapy sessions will happen and the kind of work / activities that people are advised to do between sessions.
The team can work more intensively, for up to six weeks, with people who have been discharged from an acute hospital as part of an Early Supported Discharge (ESD).
