Rehabilitation on Wye Ward
Rehabilitation supports re-learning how to do day-to-day activities following a stroke:
- You will be assessed by a variety of healthcare professionals to find out what you used to do before you had the stroke and how it has affected you since.
- The aims or goals of rehabilitation are then discussed and agreed with you – both for the long term and for whilst you are in hospital.
- You will be provided with exercises and opportunities to practice skills between your therapy sessions.
- Your progress is monitored regularly to make sure you are on track and goals can be changed accordingly.
- Rehabilitation is not just our responsibility. To achieve your goals, it is important to follow any recommendations provided by the stroke team and practice the prescribed exercises.
