What happens after I leave hospital?

If you have any concerns or queries in the days following your discharge, please call your GP, they will have a copy of your discharge summary and will be able to assist.

If further therapy is needed after your stay in hospital then a referral will have been made to our community stroke therapy team. Dependent on your needs this will either be a referral for early supported discharge or for ongoing community stroke team input. You will be made aware of this before discharge.
